completed to bring with you for your appointment. This will save you
time on the day of your appointment.
SERVICES
To provide our patients with the highest quality surgical care available, our practice is limited to the evaluation and treatment of general surgical conditions.

Esophageal reflux (heartburn), including laparoscopic surgery. |

EMERGENCIES
If you have an emergency during office hours, please call the office. If you have an emergency after hours or on the weekend, you may reach the doctor on call for the practice by calling the hospital operator at 770~535~3553. Please limit after hours calls to emergencies.
If you are experiencing a life-threatening emergency, go immediately to the nearest hospital emergency room or call 911.
PRESCRIPTIONS
We accept requests for routine prescription renewals during office hours. Please call for renewals at least two days before your prescription runs out.
BILLING & INSURANCE
We accept Medicare and most managed care and insurance plans. Please be sure to bring your insurance, Medicaid or Medicare cards with you to your appointment.
Payment is expected at the time of your visit for insurance co-payments, deductibles and co-insurance amounts. We accept check, cash, MasterCard and Visa.
As a convenience to our patients, we will file a claim to your insurance company. With participation plans, we will accept assignment of payment directly from your insurance company. Following receipt of payment from you insurance company, we will send you a statement for any remaining balance. We ask that you pay the balance within 30 days.
